By specifying your wishes regarding your digital legacy, you can ensure that your wishes are documented and that your loved ones have the necessary information to manage them appropriately after your passing.

Follow these instructions as you complete this worksheet:

• This worksheet should be completed individually by every adult completing the CLEAR Kit. If you are completing this CLEAR Kit with your spouse or partner, please have one adult complete this worksheet under “Adult Profile 1” and the other under “Adult Profile 2.” 

• Buried in Work has provided instructions on how different companies close their users’ online accounts here.

• Be sure that you have kept in mind all of the online accounts you listed in the “Online Accounts” worksheet.

• Write “N/A” when applicable.

Additionally, consider the following questions as you conduct your annual review of this worksheet:

• Have you changed your preferences regarding your digital legacy?

• Have you communicated these changes in your will or in another location?

• Is your digital legacy manager aware of their responsibilities, and do they have access to all relevant online accounts?
